Problem the AI is intended to solve
VA care teams often struggle to access Community Care documents in a timely manner. When access is granted, the process is labor-intensive, requires significant manual administration by clinical staff, and is not always successful. Veterans may also be asked to bring in a copy of their records when VA providers are unable to obtain them. This negatively impacts Veterans’ care coordination and makes them lose trust in VA.
Expected benefits
Increased efficiency in coordinating VA community care documentation, enhancing Veteran satisfaction with care and increasing quality of care and Veteran safety.
System outputs
Requests for community care records or retrieval and ingestion of community care records.
